Your own coordinator
At Amaroo, we’re here to take the stress out of managing your home care package and care needs. When we partner with you, we will introduce you to your own coordinator. They’ll get to know you and your goals and will help you make the most out of your funding.Whether you’re focused on your health or happiness, or home modifications, our range of services cover all the essentials – plus a few more you might not have thought of!
Your coordinator is there to:
- Work with you to develop your individual support plan,
- Liaise with health care professionals to ensure your needs are being met to the best of our ability,
- Review your support plan if you feel your needs are changing,
- Be your primary contact should you have any concerns or queries.
Not to mention, all our coordinators at Amaroo are also nurses. This means that your coordinator can assist you with clinical care, including nursing assessments, wound care, and catheter care.
How to access Amaroo@Home
1. Check your eligibility
To find out if you’re eligible for subsidised home care, contact My Aged Care on 1800 200 422. The Aged Care Assessment Team will ask you a series of questions to understand whether you require an at-home assessment.
2. ACAT Assessment
If deemed necessary, the Aged Care Assessment Team will contact you to organise your assessment. They will determine the level of home care that you’re eligible for, and whether you’re able to access the Federal Government’s Home Care subsidy.
3. Means testing
Amaroo@Home care packages are means tested. You must complete and lodge an Aged Care Fees Income Assessment (SA456) form with the Department of Human Services to confirm if any means testing applies.
4. Package allocation
If you’re eligible for home care, you will be placed in a national queue. You’ll be notified when an appropriate care package is allocated to you, and you will be provided with a referral code. Simply provide this referral code to us and then you’ll be allocated an Amaroo@Home package.
Referral & Receiving Service
Simply provide Amaroo with this referral code, which notifies us that the Home Care Subsidy can be applied to your fees. You will then be allocated an Amaroo@Home care package.
Processing time
Please allow ample time between contacting us and receiving your referral. If you need immediate support, then contact us as soon as possible and we’ll look into interim arrangements.
Fees and charges
We have a flexible structure in place for fees and charges to ensure that your Amaroo@Home care is affordable and manageable. We’ll run through these in a meeting with you and discuss the personalised service you’ll receive.
